Amritsar, Jan.15, 2024:- A 70-year-old man based in the US was experiencing frequent painful urination, blood in his urine (hematuria), along with acute pelvic pain. His medical examination revealed high serum PSA value, pointing towards localized prostate cancer and a left renal tumour. The case assumes significance as the occurrence of a double malignancy is extremely rare. The Patient visited several hospitals, both in the US and in India, and after exploring all options, he finally approached Dr Rohit Dadhwal, Consultant, Department of Urology, Andrology and Robotic Surgery, Fortis Hospital Mohali, recently.
Dr Dadhwal investigated the case thoroughly and after reviewing all investigations, the Patient was planned for a simultaneous robot-assisted Radical Nephrectomy (complete removal of a kidney) with Radical Prostatectomy (complete removal of the prostate gland). Given the complexity of performing both major surgeries concurrently, the primary challenge was to completely excise the tumours while minimising blood loss.
Dr Dadhwal meticulously performed Robot-assisted Left Radical Nephrectomy with Robot-assisted Radical Prostatectomy using the world's most advanced 4th Generation Robot – Da Vinci Xi. Robot-assisted surgeries are considered as the gold standard treatment in such complicated cases. Through this sophisticated procedure, DrDadhwal successfully removed the patient's left kidney tumour and prostate cancer simultaneously.
Discussing the case, Dr Dadhwal, said that cases like these where we find two tumour associations, especially prostate cancer and kidney tumour, are rare. It is a further rare occurrence when both the tumours are localised, implying that they haven't affected the other organs. In this patient, as both the tumours were confined to the organ of origin, we had the opportunity to completely remove the pathology and make him cancer free. Doing an open or laparoscopic surgery in these cases is challenging as the operative field is quite small and it takes more anaesthesia time leading to more blood loss and more complications. Fortunately, with the help of robot-aided surgery, not only were we able to remove both the tumours, but also ensured minimal blood loss.
The patient was up and walking on the next day of the surgery and his histopathology report confirmed complete removal of the left kidney and prostate carcinoma with negative margins. He was discharged on the fifth day and is leading a normal life today.
Shedding light on robot-aided surgery, Dr Dadhwal further said, "Robot-aided surgery is the latest form of minimally invasive surgery and provides a 3D view of the operative field via a special camera inserted into the body of the patient. This advanced technology allows access to parts of the body that are difficult to reach with the human hand, using robot-assisted arms that can rotate 360 degrees.
Dr Dadhwal added that we are part of a select group of centres n India and globally that have the expertise and technology to conduct such intricate robot-aided surgeries.
