GMCH Starts Registration Process for Haemophilia Patients

Chandigarh 28th July:- Govt Medical College & Hospital, sector-32, Chandigarh has started the registration process for Haemophilia patients. These patients shall submit a photocopy of the OPD card with prescription, completed Haemophilia Registration Form (available in the Haemophilia clinic), document of residence proof (Voter Card, Aadhar Card, Driving licence/telephone bill/electricity bill) & two recent photographs to Nodal officer in the Medicine OPD, Room No. 5303, Level 5, Block-B, GMCH-32, Chandigarh on every Wednesday from 10:00 am to 12:30 Noon.
